3. Have your Podiatrist or Medical Doctor fax in what pads you want/need

This will only be acceptable on your doctors RX pad and if all the below information is provided
• the exact pad and pad type must be indicated
• material must be indicated: felt, foam, moleskin or gel
• thickness must be indicated
• Patients name, address, phone number
When you or your doctor faxes in this RX the customers credit card number, expiration date, and
billing zip code of where the statement for the card goes to must be provided.
